Employee Assistance Program (EAP)

Welcome to StressCare Behavioral Health’s Employee Assistance Program (EAP).   Our EAP is a comprehensive, traditional EAP, specializing in brief, solution focused help in the moment. Our services include, but are not limited to, the following:

We are conveniently located in West Toledo and we have affiliate providers outside our service area.  Our service is completely confidential and is delivered in a private setting by a highly skilled EAP team, each of whom averages 25 years in the EAP field, they are passionate about EAP and strive to help employees resolve their challenges, so they can remain productive and healthy employees.

Rachel Chandley, PhD

Dr. Chandley graduated from Miami University’s doctoral program in 2016. She has worked extensively with individuals across the lifespan including conducting psychological assessments and providing individual and group therapy to children, adolescents, and adults. She has worked in outpatient, hospital, community mental health and school settings. During her APPIC-accredited internship at University of Cincinnati Counseling and Psychological Services she received specialized training in working with survivors of sexual and interpersonal violence. She aims to work collaboratively with her clients to help them reach their goals. She feels passionately about outreach and consultation and has interest in working with individuals from diverse ethnic, racial, sexual orientation and gender identities.

Diane Derr-Lewis, PhD

Dr. Lewis is a Clinical Psychologist with over 30 years experience providing psychological services to a variety of clients. She has worked in inpatient and outpatient psychiatric settings, nursing homes, and acute care medical centers. She has provided individual, marital, and group psychotherapy, as well as psychological assessments. She has also performed specialized assessments related to Bureau of Workers' Compensation issues. Dr. Lewis has special interests in working with chronic pain, behavioral medicine, spirituality, end of life issues, women's issues, depression, and anxiety. She has served as Director of a mental health center psychology department and President of the Board of a local American Cancer Society unit. She is licensed in Ohio and is on most insurance panels.

Marsha Drees, MSSA, LISW-S, LICDC-CS

Marsha has been practicing as a social worker and counselor for 30 years.  She earned her master’s degree in applied social science from Case Western Reserve University and her Bachelor of Art degree in social work from the University of Toledo.  She has worked extensively as a program director and clinician in community mental health and private practice settings.  She is highly skilled in the EAP model and supervisory consultation.  Her passion is helping clients overcome trauma and addiction, so they may live full lives and work productively.    

Carol Grady MS, LICDC, LSW

Carol has been practicing as a counselor for 27 years.  She earned a Master of Science degree from Dayton University and a Bachelor of Arts degree from Kent State University. She has administrative and counseling experience, having worked in community mental health, college, and EAP settings during her career.   She is highly skilled in the traditional EAP model, with an emphasis on chemical dependency counseling and DOT/SAP evaluations and training.

Kristen Holder, MSW, LISW

 Kristen has been practicing as a social worker for four years.  She earned her master’s degree in social work from Spring Arbor University, and her Bachelor of Social Work degree, with a minor in criminal justice from Spring Arbor University.  She has worked in outpatient settings as a counselor and an administrator, with an emphasis on the treatment of substance use disorders and mental health disorders.

Carol Bristol Lydy, LPCC, LICDC, LSW

Carol has been practicing as a counselor for nearly 20 years.  She has completed post graduate studies at the University of Toledo, Heidelberg College and Lourdes University.  She has a Master of Education in Community Counseling from the University of Toledo, and a Bachelor of Art degree from the University of Toledo.  She has worked in both community mental health and EAP settings during her career.   She enjoys working onsite with various EAP customers, providing EAP expertise.

Nancy K. Rank M.Ed, LPC   

Nancy has been practicing as a counselor for 26 years.  She is a graduate of the University of Toledo’s Master of Education Guidance Counseling Program and Bachelor of Education Program.   She has worked in diverse settings, including courts, hospitals, outpatient counseling centers and EAP’s.   She is highly skilled in the traditional EAP model.

Lisa F. Rectenwald, MRC, LPC

Lisa has been practicing as a counselor for 29 years.  She is a graduate of Bowling Green State University’s Master of Rehabilitation Counseling Program and the Ohio State University’s Bachelor of Psychology Program.   She has worked in a variety of settings as an administrator and a counselor, from community mental health, to inpatient chemical dependency, to EAP settings, where she has spent most of her years in practice.  She has a very calming presence and brings a holistic approach to her EAP clients.   She is highly skilled in the traditional EAP model, with an emphasis on anxiety treatment.

Glenn I. Swimmer, PhD, President and CEO

Dr. Swimmer is a clinical psychologist with experience in behavioral health and pain management. He has been in practice for more than 25 years and is the president of StressCare and the Director of PainCare of Northwest Ohio. Dr. Swimmer has published research in the area of chronic pain. He is currently on the Board of Directors of the Ohio Pain Initiative and the Northwestern Ohio Chapter of the National Multiple Sclerosis Society. Dr Swimmer has served as an Adjunct Faculty Member of the University of Michigan Physical Medicine and Rehabilitation Department, as well as on the Board of the Midwest Pain Society. He was invited to speak on Pain and Addiction for the Ohio Psychological Association, and to participate in an "Expert Roundtable" on chronic pain sponsored by the Columbus Medical Association.
